Looking at the bitrates for the subtitles, There are three full German subtitle streams, which I assume are for the film's dialogue and two commentaries. There is a small German subtitle track which I assume is for signs and text. But you say there are "German" and "German HoH" subtitles. Is this what is written in the menu? Could you check the subtitle tracks?

Judging from OFDb, I would guess the following:

1. The three “full” German subtitle tracks are for the movie dialogue (HoH or not) and for the two audio commentaries.
2. The “full” English subtitles are for the movie dialogue (HoH or not).
3. The “small” English subtitles are for the sequence where they speak Spanish (the drug lord), that is, they are not burnt-in in this transfer.
4. The “small” German subtitles are for the sequence where they speak Spanish even during the German dub which is not otherwise subtitled (perhaps also signs).
