I take it the extras are on the Blu-ray disc. Is the Blu-ray extras only or does it have the film on it as well? Meaning, is it a UHD with a bonus Blu-ray or a UHD/Blu-ray combo?

Yes the extras are on the 1080p BD50 disc with the 4K alone being on a BD66, I'm just not sure if the former is just 1.33:1 alone or also has 1.85:1...

Resurrecting an "added" thread here, but the information John provided was completely wrong. The UK 4K disc has completely different audio and subtitles, differing extras from what was listed above.
